In the early development of open heart surgery midsternotomy was not popular. Once it became obvious that midsternotomy was possible and provided excellent exposure to all parts of the heart, it became the incision of choice. However, a right thoracotomy was used in early procedures for mitral valve surgery and has now become popular once again for cosmetic reasons.
