As described earlier, the choice of a substitute depends on aortic annuls diameter, aetiology and age. A root diameter less than 30 and young (less than 35) patient with rheumatic heart disease is the ideal candidate. The decision to use a homograft is made on transesophageal echocardiography and is subject to ready availability of an appropriate sized homograft.
